左心房高压造影在心房颤动冷冻消融中的应用

摘要:

目的 探讨左心房高压造影在心房颤动冷冻消融中应用的可行性和安全性。方法 对30例首次接受导管消融治疗的心房颤动患者进行冷冻消融。穿刺房间隔后,通过Swartz L1导管连接高压注射器对左心房及肺静脉进行左前斜45°和右前斜30°的双体位非选择性造影。高压注射参数为流速15 ml/s,总量30 ml和最高限压600 lb/in2。结果 无论左心房大小,左心房高压造影均可清晰显示心房及肺静脉的解剖结构和解剖关系。未发生与高压造影相关的并发症。结论 在心房颤动冷冻消融中,应用高压造影了解左心房和肺静脉解剖信息是安全可行的。

Abstract:

Objective To explore the feasibility and safety of applying high-pressure angiography in the left atrium during cryoballoon ablation of atrial fibrillation. Methods This study enrolled 30 patients with atrial fibrillation treated by cryoballoon ablation. After the atrial septum was punctured,high-pressure angiography was performed in the left atrium by connecting a high-pressure injector to a Swartz L1 catheter. This method aimed to obtain anatomical information about the left atrium and pulmonary veins using two projection positions: left anterior oblique at 45° and right anterior oblique at 30°. The parameters for the high-pressure injection were a flow rate of 15 ml/s,a total volume of 30 ml,and a maximum pressure limit of 600 lb/in2. Results High-pressure angiography clearly displayed the anatomical structures of the left atrium and pulmonary veins,regardless of the left atrial size. There was no complication assosiated with high-pressure angiography. Conclusion High-pressure angiography in the left atrium is feasible and safe during the cryoballoon ablation procedure for atrial fibrillation.
